In many men with vasculogenic erectile dysfunction, reduced penile blood flow contributes to difficulty achieving or maintaining erections. Li-SWT is a non-invasive treatment that delivers low-intensity shockwaves to erectile tissue, aiming to stimulate biological responses that may improve penile vascular function and erections.

What do the European Urology Guidelines say?

The 2026 European Association of Urology (EAU) Guidelines on Sexual and Reproductive Health state that “Low-intensity shockwave therapy can induce a mild improvement in erectile function among patients with vasculogenic erectile dysfunction (Level of Evidence 1a).”

Use low-intensity shockwave treatment (Li-SWT) with/without oral PDE5Is in patients:

  • with mild vasculogenic erectile dysfunction;
  • as an alternative therapy in well-informed patients who do not wish to have or are not suitable for oral vasoactive therapy;
  • who are vasculogenic ED patients that are poor responders to PDE5Is.

Are all “shockwave” treatments for ED the same?

No — and this is important.

The term shockwave therapy is sometimes used for technologies that deliver very different types of acoustic energy.

Radial pressure waves spread outward from the applicator and predominantly act superficially, which does not help in erectile dysfunction.

Focused shockwaves, in contrast, concentrate acoustic energy within a defined treatment zone at a selected tissue depth. Most clinical evidence supporting Li-SWT for erectile dysfunction has been generated using focused shockwave technology.

Before undergoing treatment, patients should therefore ask not only “Do you offer shockwave therapy?” but also “What type of shockwave technology is being used?”

What makes PiezoWave2T different?

At Orchidz Health, we use the PiezoWave2T, a premium German-engineered system from Richard Wolf and Elvation.

It generates focused shockwaves using piezoelectric technology and, importantly, offers an additional linear-focused therapy source.

How does the shockwave help in living tissue?

Extracorporeal shockwaves are mechanical stressors that can induce biochemical changes in living tissue; this process is called Mechano-transduction.

Scientifically described mechanisms:

By placing the therapy source at right angles to the course of the corpus cavernosum, the linearly focused shockwave of the PiezoWave2T makes it possible to treat the entire course of the corpus cavernosum quickly and comprehensively, ensuring maximum, homogeneous energy delivery to the target tissue.

Richard Wolf and Elvation from Germany developed this concept for erectile dysfunction as LSTC-ED – Linear Shockwave Tissue Coverage for Erectile Dysfunction.

Who may benefit from low-intensity shockwave therapy (Li-SWT)?

Li-SWT is primarily considered for appropriately selected men with vasculogenic ED, particularly mild erectile dysfunction. It may also be considered in selected men who have an inadequate response to medicines such as sildenafil or tadalafil.

Shockwave therapy is not suitable for every type of ED. Identifying the underlying cause and selecting the right patient are essential.

What happens during shockwave therapy?

Treatment is performed as an outpatient procedure without injections or anaesthesia. Shockwaves are delivered to specific areas of the penis (corpora cavernosa and crura) according to our clinical protocol.

How many sessions of shockwave therapy are required?

The number of sessions varies across centres depending on various patient characteristics. At Orchidz Health Bengaluru, our treatment course consists of a minimum of 4 sessions, usually one session per week. Each session takes approximately 10–15 minutes, and patients can generally return to their routine activities immediately. One can pay per session each week or take advantage of a discount if you pay upfront for 4 sessions

How do I know if I am suitable for shockwave therapy for ED?

Treatment begins after consulting our Uro-Andrologist. Further evaluation, including hormonal assessment or penile Doppler ultrasound, may be advised when clinically appropriate.
